NDIS - Student Intake Form

Thank you for being a part of our studio!

If you would like us to directly bill your NDIS plan manager, or be provided an itemised invoice, please fill in the form below.

Please note that our terms and conditions require up to 14 days before your next billing date to cancel or make changes to ongoing lesson times and auto-debit payments, although we usually attend to these requests as quickly as we can. This allows us to ensure scheduled payments are given ample time for system changes to occur, and for your teacher schedules to be updated.​ Enrolment pauses (where billing is suspended) are allowable for periods of 1 month or over.
